The terms and conditions below apply to any person receiving services/treatment by A.R.M Physiotherapy and Massage. These must be agreed to before the commencement of any treatment.
1. Services and Assessments
1.1. Services and treatment provided must be agreed upon and signed for by the patient during the initial assessment appointment.
1.2. Assessments require discussion of patient needs and injury concerns. All patients must undergo an initial assessment before any injury treatment.
1.3. A health screening questionnaire and informed consent form must be completed before beginning treatment.
1.4. Follow-up appointments will be agreed upon by both practitioner and patient.
1.5. Treatment may include both ‘hands-on’ techniques and exercise rehabilitation, depending on the injury, as advised by the practitioner.
1.6. All treatments remain within the practitioner’s professional scope of practice. Where appropriate, the practitioner may recommend an alternative provider if care required falls outside of this scope.
1.7. The practitioner reserves the right to terminate a session at any time if deemed necessary. In such cases, the patient may still be liable for payment at the practitioner’s discretion.
2. Payments
2.1. Payment must be made at the time of the appointment or in advance via card, cash, or BACS, payable to A.R.M Physiotherapy and Massage.
2.2. Charges for home visits vary based on treatment needs and travel time. Payment can be made via the same methods as above.
3. Cancellations and Late Attendance
3.1. If you arrive late, your session will still end at the scheduled time, and the full appointment fee will apply.
3.2. A £15 deposit is required to secure your booking. Cancellations or rescheduling must be made at least 24 hours in advance. Cancellations made within 24 hours of the appointment will result in the loss of the £15 deposit.
3.3. Failure to attend your appointment without notice (“no-show”) will be charged at the full appointment cost.
3.4. If the practitioner needs to cancel an appointment (for example, due to illness), your session will be rescheduled at the earliest available opportunity.
3. Confidentiality and Data Protection
4.1. Patient information will be treated confidentially at all times. Information will only be disclosed if required by law or for the safety of the patient or others.
4.2. Personal and medical information is collected solely for the purpose of providing appropriate care and managing appointments.
4.3. All data is stored securely in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.
4.4. You have the right to access, correct, or request the deletion of your personal data. For more details, please refer to our full Privacy Policy.
5. Home Visits
5.1. For home visits, the therapist may need to bring equipment. Space requirements will be discussed in advance.
5.2. For payment terms, please refer to section 2.2.
All services at A.R.M Physiotherapy and Massage are supported by professional accreditation and qualifications.
